The Biopsychosocial Perspective on Low Back Pain with Professor Stuart McGill
If you have back pain, you might have been told "it's all in your head". This type of comment can be disempowering and unhelpful. At the same time, however, your brain and psychology do affect your experience of pain, as promoted by advocates of the "biopsychosocial model".
In this video, Professor Stuart McGill uses a powerful example from his clinical practice to explain his thoughts on this subject.

    Hello, I have a question regarding core engagement. I just recently had a microdiscectomy (5 days ago). And I had previously learnt how to engage my transverse abdominals correctly. My core is at its strongest it’s ever been and have learnt how to engage pretty much all the muscles around my torso (obliques, rectus abdominis, internal and external obliques). I wanted to know how much I should be engaging my core at this time and if there is any danger in engagement at all? I was worried I would put too much compression on my lower back by too much engagement. Or is it good that I am engaged, I’ve heard it supports your back like a internal corset. Could you advise please?

    I’m reading Back Mechanic and listening/watching every McGill podcast & video I can find that mentions Spondylolisthesis. If anyone at Backfitpro Pro reads the RUclips comments, I would love any links or suggestions for where I should go to learn more! My “Spondy” is stenosis related - not injury related. Cheers!

    Oh I’m so glad to have found this. I’m reading the back mechanic book & just couldn’t understand what this was all about! I am 54 with spondylolisthesis, stenosis ….. standing is so hard, sitting feels great …. I want to stay active & help my grumpy back …. am learning loads from McGills many books and podcasts Thank you for all the education and knowledge shared!

    Awesome! To add body adapts tention may travel up. I had years since my body learned how to block lower back pain in the cost of upper body chronic stiffness. It's may be a good price untill it goes up to the neck tentions. You can bring back upper body mobility in the cost of lower back pain. Only strong core may fix it. Hopefully.

    There was a NASCAR driver who used the same principle of stiffness to race from 10th to 5th in the final lap... he jammed the side of the car against the wall and gunned it. The stiffness of the wall kept his car under control so he could keep the pedal to the metal as he went around the oval. He zoomed past everyone, though the move is now considered illegal (it demolished the side of his car too)

    Pain and Injury is the great corrupter! Truer words never spoken. I think its also true for one's mental state. I'm sixty eight and its a shame this knowledge was not readily available or even known forty years ago. Just awesome content.

    Couldn’t we explain the injury during fluoroscopy as a pain inhibition from excessive muscular strain of the paraspinals allowing for the flexion at 2-3? I find a good way to tell the difference on exam to be a supported lumbar flexion. If you support the upper body upon flexion and the pain goes away, it’s muscular. If the pain persists, it’s discogenic. The visceral innervation also tends to give it a more diffuse localization. Overall, I wonder if we over attribute pain to structural abnormality vs neuromuscular dysfunction. Oh no, I necro’d the thread. Sorry internet.

    Am i supposed.to be feeling my muscles in the back working as in tightining when doing the big 3? Im not extending or flexlion of my spine. Just feel the in my back working is that ok?

I have been to so many specialists and they all only say that it is something degenerative with no solution.     I've been a full time farrier (horseshoer) for 26 years and for the better part of that time I have endured debilitating lower back pain. I've always just gritted my teeth and suffered through it. I'm learning a lot from these videos and have ordered the back mechanic book. I think my life has been a recipe for back pain. I was a 4 sport athlete through high school (football, basketball, track, wrestling). I hauled hay at nights during my teen years working on a hay crew and we lifted and stacked 80lb bales at a rate of about 1800 bales a night. I got obsessed with upper body weight training after I broke a bench press record in high school, but I totally ignored any lower body training. I then worked on a ranch where my job was to break horses. I broke and trained over 400 horses while working there, training most of them to be sold as polo ponies... then I attempted a short career playing polo professionally before giving that up to shoe horses full time. I've shod over 70,000 horses, worked 7 days a week most of my life. Raised 4 kids. Now I'm as stiff as the tin man, my back is shot, when I bend over working I often have to find a wall or something to pull up on so I can stand up. My back muscles fatigue very quickly. Sometimes it's all i can do not to fall forward on my face when I bend over. My lower back slips out if I sneeze, if I step off a curb, or out of a truck, or if I have to react to something quickly, After work I lay down and can't get anything done around the house because I need every bit of back recovery I can manage to put towards the next day's work. I have a vertebra that feels out of place as I can put my thumb in the space where I believe it should be. I'm starting to recently feel what seems to be fluid or soft swelling around that specific area. I bought a tens unit and that thing feels like it is pulling my vertebra out and in, it seems to relax the muscles but the next morning after I do the tens unit my back pain is far worse. I'm really hoping Dr McGill's exercises help me and looking forward to the book arriving. I've briefly tried a few of them and feel as though they are helping,,, almost like a warm sensation and a brief erasure of all pain after just attempting to do the few things I've found him mentioning online.

    I know you're the low back pain guy... but my issues are terrible upper back and cervical instability. I'm wondering if you have any recommendations for exercises to remedy these issues, or if I should just focus on the 'Big 3'. I've tried a great deal of things and I seem to still be getting worse. I'm starting on 'The Big 3' but I seem to keep hurting myself doing curl ups and level 1 side bridges. Always yank something in the back of my head.

    I was a student at the University of Waterloo in 1986 to 1989 and had a young professor with a very impressive mustache! His name was Stuart McGill. Little did i know that he would become a workd-wudecsuthirity in the management of low back pain. Now, over 30 years later I am a chiropractor in Bangalore. India and still learning from my professor via RUclips.

    Quite a random observation. Wanted to share nonetheless. Ever since starting with Big 3 I've found that my hands and feet are warmer. I often had cold hands and feet (came in waves). Not sure if you've come across this before? Anyways, cheers.

    Prof. McGill's method put me back upright after a huge L5 and spondyretro listhesis injury ( the alternative was screws and rods in my back - still a looming promise of a neurosurgeon ) ...then I moved on to calisthenics, the injury was a distant memory, became overconfident/cocky in my training, neglected the big three routine and 13 years later ...it caught me out out again with a vengance.. Now I'm out of it again only thanks to reintroducing the big three routine ...As he says , not once or thrice a week but everyday.. like a prayer.. for life, Thank you Profesor...

    not sure if I'm understanding correctly. Did Dr McGill recommend ditching the straps? As in, no straps when deadlifting? Does that mean you'd do alternate grips? hook grip? Isn't there a point when the grip becomes the bottle neck?

      Double overhand to lock in the lats and get symmetry. Then hook grip for heavy sets. Pick your battles at higher weights.

    What about stenosis caused by a bulged disk? The treatments for each seem counter productive to each other? How do we resolve this concern?
